Heart failure is the only majör disorder which increases the prevalance and incidence among cardiovascular diseases leading to morbidity and mortality that are commonly observed in men and women all over the world. In heart failure, which is the last stage of cardiovascular diseases, stability fails in 50% of the patients in acute decompensation periods. Nurses have a quite effective role in improving the care and life quality of the patients with heart failure. The aim in care is to improve the results of the patients and to decrease the hospitalisation, duration of hospital stay, cost and mortality. In disease management approach which can prevent heart failure decompensation, nurses should consider the current guidelines to apply the pharmacologic and nonpharmacologic interventions. In this artcile, evidence-based care recommendations have been proposed in accordance with the fundamental principles of heart failure management programs.
