The Nursing Care to COPD Patient with/who had Mitral Stenosis: Case Report

Chronic obstructive pulmonary disease (COPD) is defined as a preventable and curable disease, which is characterised by narrowed airways and airflow limitation. COPD, which is a major cause of chronic morbidity and mortality all over the world, constitutes a significant public health problem with the cause of serious job dismissal and economic burden. COPD often coexists with other diseases that have may a significant impact on prognosis. Comorbidities could impact on symptomology, quality of life, the complications, the management, economic burden and the mortality of the disease. Nursing care for COPD patients is to relieve symptoms, improve treatment regimen, and improve quality of life to prevent complications. In order to increase the effectiveness of nursing care, the evaluation of the patient with the holistic approach and the continuity of patient care are important. A 69-year-old male patient with increasing complaints of palpitation and shortness of breath of one month duration contacted the emergency service of a state-run hospital. Lung sounds were considered harsh and prolonged in expiration. He was COPD exacerbation, pneumonia, AF with rapid ventricular response despite pharmacological treatments, emergency echocardiography was required for the patient due to increasing shortness of breath and continuous palpitation. The patient was diagnosed with mitral stenosis as a result of emergency echocardiography and admitted to the cardiology service for further evaluation and treatment. The case was evaluated with Marjory Gordon's Functional Health Patterns Nursing Care Model and nursing interventions were applied and the results were evaluated.

Mitral Stenozlu KOAH Hastasında Hemşirelik Bakımı: Olgu Sunumu

Kronik obstrüktif akciğer hastalığı (KOAH), solunum yollarında daralma ve hava akımı kısıtlanması ile karakterize, önlenebilir ve iyileştirilebilir bir hastalık olarak tanımlanır. Tüm dünyada önemli bir kronik morbidite ve mortalite nedeni olan KOAH, ciddi bir iş görmezlik ve ekonomik yüke yol açması nedeni ile önemli bir halk sağlığı sorununu oluşturmaktadır. KOAH’da komorbiditeler sık görülür ve bunların prognoz üzerinde önemli etkileri olabilir. Komorbiditeler, semptomları, yaşam kalitesini, komplikasyonları, hastalığın yönetimini, ekonomik yükünü ve mortalitesini etkilemektedir. KOAH'lı hastalara yönelik hemşirelik bakımı, semptomları iyileştirme, komplikasyonları önlemek için tedavi rejiminin uygulanması ve yaşam kalitesinin arttırılmasıdır. Hemşirelik bakımının etkinliğini arttırabilmek amacıyla holistik yaklaşımla hastanın değerlendirilmesi ve hasta bakımının sürekliliği önemlidir. Altmış dokuz yaşında erkek hasta son bir aydır çarpıntı, nefes darlığı şikayetinin artması nedeni ile bir devlet hastanesi acil servisine başvurdu. Akciğer sesleri kaba ekspiryum ve uzun oskülte edildi. KOAH alevlenmesi, pnömoni, hızlı ventrikül yanıtlı atrial fibrilasyonu olan hastaya yapılan farmokolojik tedaviye rağmen çarpıntı şikayetinin geçmemesi, nefes darlığının artması üzerine acil EKO’su istendi. Acil EKO’sunda mitral stenoz saptanan hasta ileri tetkik ve tedavi için kardiyoloji servisine yatırıldı. Olgu Marjory Gordon’un Fonksiyonel Sağlık Örüntüleri Hemşirelik Bakım Modeli ile değerlendirilerek, hemşirelik girişimleri uygulandı ve sonuçları değerlendirildi.
