Koroner Yoğun Bakımda Kalp Yetersizliği Hastaları İçin Belirlenen Hemşirelik Tanıları ve NANDA’ya Uygunluğunun Değerlendirilmesi

Amaç: Bu çalışma, koroner yoğun bakım ünitesinde (KYBÜ) yatan kalp yetersizliği hastalarının hemşirelik bakım planlarında yer alan hemşirelik tanılarının belirlenmesi ve bu tanıların Kuzey Amerikan Hemşirelik Tanıları Birliği’ne (NANDA) uygunluğunun değerlendirilmesi amacıyla yapıldı.
Yöntemler: Çalışmanın örneklemini, 1 Ocak 2015-1 Ocak 2016 tarihleri arasında KYBÜ’de kalp yetersizliği tanısı ile yatan 189 hastanın hemşirelik bakım planı oluşturdu. Tanımlayıcı ve retrospektif nitelikte planlanan çalışmada kategorik değişkenler sayı ve yüzde, sürekli değişkenler ortalama ± standart sapma olarak belirtildi. Tanıların uygunluğu, NANDA 2009-2011 tanı listesine göre belirlendi.
Bulgular: İncelenen hasta dosyalarında hastaların yaş ortalaması 65,5±13,1 olup; KYBÜ’de ortalama yatış gün sayısı 7,3±6,9 bulundu. Hastaların sahip olduğu ek hastalıklara bakıldığında (%85,7); en fazla diyabet (%58,2) ve hipertansiyon (%58,2) tanıları tespit edildi. Kayıtlarda yer alan hemşirelik tanıları incelendiğinde, toplam 30 farklı hemşirelik tanısı saptandı. Kaydedilen 1053 tanıdan %90,4’ünün (n=951) NANDA’ya uygun olduğu belirlendi. En fazla kaydedilen hemşirelik tanılarının “enfeksiyon riski” (%96,3), “öz bakım eksikliği” (%88,4), “bilgi eksikliği” (%85,2) ve “kanama riski” (%84,1) olduğu saptandı. NANDA’ya uygun olmayan yedi tanı (hiperglisemi, hipoglisemi, hipertansiyon, ritim bozukluğu, kardiyak tamponad, emboli ve digoksin intoksikasyonu) tespit edildi.
Sonuç: Bu çalışmada, belirlenen hemşirelik tanılarının yüksek oranda (%90,4) NANDA’ya uygun olduğu görüldü. Ancak; hemşirelik bakım planlarının yeterli olmadığı ve hastalığa özgü bazı tanıların ise göz ardı edildiği söylenebilir.

Nursing Diagnoses for Heart Failure Patients in Coronary Care Unit and Evaluation of NANDA Compliance

Objective: This study was conducted to determine the nursing diagnoses of nursing care plans of heart failure patients in coronary care unit (CCU) and to evaluate the compliance of these diagnoses with the North American Nursing Diagnosis Association (NANDA).
Methods: The sample of the study was consisted of nursing care plans of 189 patient who had been admitted to the CCU with diagnosis of heart failure between January 1, 2015 and January 1, 2016. This is a descriptive and retrospective study and data was presented as numbers and percentage distribution. The compliance of the diagnoses were determined according to the NANDA 2009-2011 diagnosis list.
Results: The mean age of the patients was 65,5±13,1 and number of the hospitalization days in the CCU was 7.3±6.9. Regarding the co-morbidities; the most diagnosed were diabetes (85.7%) and hypertension (58.2%). When the nursing diagnoses in the records were reviewed, a total of 30 different nursing diagnoses were detected. Of the 1053 diagnoses recorded, 90.4% (n=951) were found to be compliant with NANDA. The most frequently recorded nursing diagnoses were "risk of infection" (96.3%), "lack of self-care" (88.4%), "lack of knowledge" (85.2%) and "bleeding risk". However, seven diagnoses (hyperglycemia, hypoglycemia, hypertension, dysrhythmia, cardiac tamponade, embolism, and digoxin intoxication) were found to be non-compliant with NANDA.
Conclusion: This study revealed that the detected nursing diagnoses were highly compliant (90.4%) with NANDA. But, nursing care plans were not sufficient and some disease specific diagnoses were ignored.
